The following incidents occurred on December 20, 2013:
At 3:02 PM, Deputies responded to the W5100 block of STH 11/81, in Monroe Township, for a single vehicle vs. deer collision. Roberta K. Perkins, 37, of Rockton, IL, was traveling westbound on STH 11/81 when a deer collided with her vehicle. Perkins and her passenger were properly restrained and were not injured. Perkins vehicle sustained moderate damage with no airbag deployment and was driven from the scene.
At 4:26 PM, Lynn P. Nimsomboon, 37, of Brodhead, was southbound on STH 69 when she lost control of her vehicle. The vehicle then entered the ditch, overturning. Nimsomboon was injured in the crash and was taken to Monroe Clinic ER for treatment of her injuries. Nimsomboon was wearing her seatbelt. The vehicle sustained severe damage and there was no airbag deployment. The vehicle was towed from the scene.
At 9:44 PM, Deputies, along with Juda Fire Department & First Response, Monroe EMS, Brodhead EMS and WI State Patrol, responded to a two-vehicle crash on STH 11, near Hartwig Road, Town of Sylvester. Investigation revealed that a vehicle operated by Tracey L. Schollmeyer, 50, of Hudsonville, MI, was westbound on STH 11 when she lost control of her vehicle. The Hyundai collided head-on with a semi traveling eastbound, which was operated by Paul T. Fantene, 60, of O'Fallon, IL. The Schollmeyer vehicle spun into the north ditch, coming to a rest. The semi unit entered the south ditch, colliding with a road sign; then coming to a rest. Schollmeyer sustained injuries and was transported to Monroe ER by Brodhead EMS. Her seatbelt was used and the airbag did deploy. A passenger identified as Sophie A. Schollmeyer, 13, of Hudsonville, MI, sustained injuries and was transported by Green County EMS to Monroe ER. Her seatbelt was used and the airbag did deploy. Another passenger, identified as Isabella M. Schollmeyer, 10, of Hudsonville, MI, sustained injuries and was transported by Green County EMS to Monroe Clinic ER. The Schollmeyer vehicle sustained severe front-end damage and was towed. Fantene sustained injuries and refused medical treatment at the scen. His seatbelt was used and the airbag did not deploy. The semi unit sustained severe front-end damage and was towed. The refrigerated semi-trailer was not damaged. Ice-covered roadways were a contributing factor in the crash. The crash remains under investigation, pending the commercial motor vehicle post-crash inspection. State Highway 11 was closed to one lane of traffic for approximately 4 hours and Green County Highway assisted.
