The following incidents occurred on May 12, 2014:

Driver's License Violation:
At 3:32 A.M., John P. Loque, 22, Monroe, was cited for Operate with a Suspended Driver’s License on STH 11, at Ullom Road, in the Town of Cadiz. Loque was released pending court.
 
Motor Vehicle Crash:
At 11:52 A.M., Deputies, Monroe EMS and Browntown Fire Department/First Response were dispatched to a motor vehicle crash on STH 81, .2 mile east of Skinner Hollow Road, Jordan Township. Investigation revealed that a car, driven by Jacquelin R. Hartwig, 83, of Argyle, WI, was traveling west on STH 81 when she lost control of her vehicle and slid into the ditch on the north side of the roadway. Hartwig's vehicle slid sideways in the ditch and struck several rocks/boulders, which caused her vehicle to overturn. The Browntown Fire Department assisted in extricating Hartwig from the vehicle and she was then transported to the Monroe Clinic Hospital by Monroe EMS for treatment of injuries sustained in the accident and was later taken to UW Hospital by Medflight. Hartwig was wearing a seat belt and no air bags were deployed on her vehicle. Hartwig's vehicle sustained severe damage and was towed from the scene.
 
Motor Vehicle Crash:
At 3:20 P.M., Deputies, Wisconsin DNR, Monroe EMS, and Juda Fire Department responded to the W2300 block of STH 11/81, west of CTH OK, in Spring Grove Township, for a motor vehicle crash. A motor vehicle owned/operated by Melissa M. Fischer, 45, of South Beloit, IL, was eastbound on STH 11/81 when her pickup truck bed cover and supports detached from the vehicle due to strong wind gusts. This caused the truck bed cover and supports to become airborne and collide with a westbound vehicle. The westbound vehicle was owned/operated by Jamie J. Muniz, 26, of Monroe. The cover and supports impacted with the front end of the Muniz vehicle, causing severe front-end damage. A support bar from the cover (roughly three feet in length) impacted with the Muniz vehicle windshield and entered the vehicle, striking Muniz in the head.  Fischer was the lone occupant in her vehicle, was properly restrained, and was uninjured. The Fischer vehicle was undamaged with no airbag deployment and was driven from the scene. Muniz was the lone occupant in the other vehicle, was properly restrained, and sustained injuries. Muniz vehicle sustained severe damage with no airbag deployment and was towed from the scene.  Muniz was transported by Monroe EMS for medical treatment.
 
Parole and Probation Warrant:
At 5:07 P.M., Deputies arrested Andy J. Markham, 38, of Juda, on a Department of Corrections Warrant at the Green County Justice Center. Markham was jailed.
